I made my own with 1/4" plate and the original engine mounts combo. They are called Spicer Plates for the guy who invented them. They're easy to make and work very well.
Here's a shot of them installed:

Did the same thing FNG did-one thing you might want to consider is what you are going to run for exhaust. I ended up with a smaller triangle shape because I was running shorty headers and with a larger plate they were touching. Just FYI before you have to start grinding on nice painted pieces because you didnt preplan-not that I would know about that...lol
i,ve got some mounts that work with stock frame horns no modification to truck at all. if it helps you.\

If the stock frame horns are gone, you can do what ever you want.
I would suggest a style that has a plate bolted to the block of the engine. Channel iron or something of similar strength running out to sit on top of the frame rails. With a round rubber mount to cusion the mount and a bolt through the mount and frame.
Remember if you didn't build it you can't call it yours.

If you can get some FSJ AMC V8 motor mounts, you might be able to find a Chevy adapter. Or, look for a Chevy adapter for the CJ's that used to have AMC V8's.
The FSJ motor mounts literally bolt in, so I'm told by the guy who did my engine conversion.
I have an 83 chevy truck I'll be scrapping soon. If you want the chevy frame mounts, lemme know. The shipping should be reasonable, the parts are free. Of course it'll require some mods to the mounts and then welding them in.
Advance adapters has the wide Jeep frame small block conversion mount kit. All you have to do is either bolt or weld the frame plate to the frame. Comes with the block mounts, the frame plates, and the rubber insulators w/bolts.
